Dear TPS Parents, Guardians, and Caregivers:

Please take a moment to fill out this form for your child to change or reconfirm your child's cohort for the rest of Trimester 2 (starting January 19 through March 19). It is our intention to undertake this same process for Trimester 3. If we are unable to accommodate your request, we will contact you. All survey requests must be completed by January 5, 2021. If you are unable to complete the survey in that time, we will consider you to be making NO change to your current cohort.
