Changes in Student Health: Injury, Surgery, or Concussion
Caregivers, please immediately notify your student's school nurse of any physical injury or changes in a student's health status. All students coming to school using crutches need to visit the school nurse right away.
A doctor's note is required if your student returns to school with any issue or condition that would limit their participation in any educational activity or sport. Doctor's notes should be submitted to your student's school nurse.
For example, if your student has:
- a new physical injury, including a broken bone or a concussion
- a health update after surgery
Excusal Notes for Physical Activities
- A doctor's note is required to return to play any sport after a medical excuse for illness or a suspected or diagnosed concussion. You can read more about state guidelines regarding concussions by clicking here.
- A parent may provide a written excuse from PE/Gym class for a maximum of two school days; after that, a doctor's note is required.
Doctor's Notes
If you need to provide a doctor's note for a student's injury or status post-surgery, please make sure it includes the following:
- Nature of the illness, injury, surgery recovery, or concussion
- Any limitations or restrictions, including physical education and recess
- The date(s) the student should be excused from any physical activity
