Changes to Share with the School Nurse
Injury, Surgery, or Concussion
Students who come to school using crutches must report to the school nurse as soon as they arrive.
A doctor’s note is required if a student returns to school with a condition that limits participation in school activities or sports. Doctor’s notes should be given to the school nurse.
Example Situations Include:
- A new physical injury, including a broken bone or a concussion
- A health update after surgery
Submit Excusal Notes for Physical Activities
- A doctor’s note is required to return to play any sport after a medical excuse for illness or a suspected or diagnosed concussion. Learn more about Massachusetts guidelines for concussion care and return to school.
- Legal guardians may provide a written excuse from wellness (physical education) class for a maximum of two school days. After two days, a doctor’s note is required.
Doctor’s Notes
If you need to provide a doctor’s note for a student’s injury or status after surgery, please make sure it includes the following information:
- The nature of the illness, injury, surgery recovery, or concussion
- Any limitations or restrictions, including physical education and recess
- The date(s) the student should be excused from any physical activity
