Giving Medication at School

When possible, medications should be given at home. If medication must be given during the school day, caregivers must follow school requirements


Access Medical Forms

Caregiver consent

A signed permission form must be submitted to the school nurse

Healthcare provider order

A written order from a licensed provider (such as a doctor or nurse practitioner) must be completed and given to the school nurse. This form must be updated as needed and at the start of each school year.

Medication in original container

Medication must be delivered by a caregiver or designated adult in a pharmacy- or manufacturer-labeled container. Please ask the pharmacy for separate containers for home and school.

Limited supply

No more than a 30-day supply of medication should be brought to school.

EpiPens, Inhalers & Other Rescue Medication

Students who use an EpiPen, inhaler, or other rescue medication must also provide this documentation and the medication.
